Pounds of Flesh is one of those seasonal currencies that doesn't feel optional. If you're pushing endgame, you're going to slam into the shortage fast, right alongside your hunt for Diablo 4 Items that actually move your build forward. It isn't a cute crafting ingredient you can ignore until later. It's a key that opens specific lairs and reward chests, and without it you're basically locked out of the loop that spits out runes, boss reagents, and the uniques you're probably still missing.

Start with seasonal reputation, because it's steady

If you want the most reliable flow, you start with the seasonal rep track. Not because it's exciting, but because it pays. Run the Blood-themed stuff that stacks reputation quickly—Infernal Hordes, Bloodied Dungeons, and whatever event chain is fastest for your character. Then cash that progress in for repeatable caches. You'll notice the pattern pretty quick: those caches tend to cough up one or two Pounds of Flesh almost every time. That "almost" matters, sure, but it's still way more consistent than hoping a random dungeon boss feels generous. If your build clears packs without stopping, you can knock out rep chunks on autopilot and keep your stash topped up.

Use Helltides as your second lane

When Helltides are up, I treat them like my backup farm that also happens to print other resources. The goal is simple: keep moving, keep killing elites, and turn cinders into Tortured Gift chests. Those chests have a solid shot at dropping Pounds of Flesh, and you're not wasting time because you're also grabbing crafting mats and gear along the way. Do it on the highest World Tier you can handle without face-planting. If you're dying every two minutes, the "better drops" don't matter. If you can cruise, you'll open more chests per Helltide and the numbers start to add up.

Turn your stockpile into boss rotations

Once you've built a cushion, spend it with intent. Some seasonal lair encounters only cost three units to start, so they're perfect for chaining. Group play helps a ton here—faster kills, less downtime, and more chances to keep the loop going through indirect refunds and extra reagents. What doesn't work? Wandering into random Nightmare Dungeons and praying. The loot pool is bloated, and outside targeted seasonal content the drop rates feel like they're set to "don't bother." Stick to activities that are clearly designed to feed the currency back into itself.

Keeping the grind from getting stale

The best rhythm is rotating: rep farms when you want consistency, Helltides when you want density, then a tight boss loop when you're ready to cash out. That mix keeps you from burning out and makes your progression feel like it's moving every session.
